Do you know what it is like to be partnered with a Care Coordinator who can help you navigate the service maze? Whether you are seasoned in the RWA service offerings or new to the jurisdication, accessing case management services, supports your awareness of what is available and what you are eligible for.

Contact any of the offices below to begin your eligibility process. Anticipate that there will be forms to be signed, paperwork for you to submit and some questions to be asked. But after eligibility is determined, you will have access to a provider network that can effectively help you handle life as a person living with HIV or AIDS.
